The Edinburgh Psychiatry Duty of Candour Policy

As a provider of an independent health care service Edinburgh Psychiatry is required to develop and implement a duty of candour policy that describes how our staff will act in the event of an unintended or unexpected incident that has happened with patients.

Our policy requires that we publish a full report of any incidents that may have occurred in the full year of the policy being enacted.

As part of our responsibilities, we must produce an annual report to provide a summary of the number of times we have triggered duty of Candour within our service.

Our Standard Operaon Procedure is available on our website and fully institutionalised in the clinic.

Incident Reporting

A person died

A person incurred permanent lessening of bodily, sensory, motor, physiologic or intellectual functions

A person’s treatment increased

The structure of a person’s body changed

A person’s life expectancy shortened

Type of unexpected or unintended incidents (not relating to the natural course of someone’s illness or underlying conditions)

A person’s sensory, motor or intellectual functions were impaired for 28 days or more

A person needed health treatment to prevent them from dying

A person needed health treatment in order to prevent other injuries (as listed above)

Management Statement

This is the Edinburgh Practice’s first year of practice, and we are delighted to say that the policy has been fully implemented and all staff have been trained on the requirements. The clinic has not had any Duty of Candour incidents within the year.

Healthcare Improvement have been notified of the publication of this report.

The next report is due at the end of January 2024.
